Filling out your SCRIP order form is very easy.
First, at the top, clearly print your name (first and last) and your phone number where you can be contacted.
There are 4 categories of SCRIP certificates available for purchase:
Community SCRIP: (These are local and area businesses that produce their own certificates for their local and area stores.)
Department Stores: (These are national franchise department store businesses who participate in our program and make SCRIP certificates available for purchase that can be used in the stores they have in our area.)
Restaurants: (These are national franchise restaurants who participate in our program and make SCRIP certificates available for purchase that can be used in the restaurants they have in our area.)
Specialty Stores: (These are national franchise specialty store businesses who participate in our program and make SCRIP certificates available for purchase that can be used in the stores they have in our area.)
Notice that after each of the 4 categories of SCRIP certificates, it says "(Enter Dollar Amount)." That means that you should enter the total dollar amount of SCRIP certificates you wish to purchase on the blank to the right of the denomination of certificate you wish to purchase. For example, if you wanted to purchase $200 worth of Family Foods/Nash Finch certificates, you would fill out the Family Foods/Nash Finch line under the Community SCRIP category like this if you wanted the certificates in $10 denominations:
Family Foods/Nash Finch (5%): $10_200_ $25______
(The number in RED represents what you would write, and where you would write it.)
(When your order is filled, you will receive 20 of the $10 certificates, equalling the $200 worth that you indicated when you filled in the blank.)
If you wanted the Family Foods/Nash Finch SCRIP certificates in $25 denominations instead, you would fill out the same line like this instead:
Family Foods/Nash Finch (5%): $10_____ $25__200_
(The number in RED represents what you would write, and where you would write it.)
(When your order is filled, you will receive 8 of the $25 certificates, equalling the $200 worth that you indicated when you filled in the blank.)
Once you have filled out the line for each of the businesses for which you wish to purchase SCRIP certificates, you need to write your signature on the "Signature" line near the bottom of the form.
You also need to add up the total dollar amount of SCRIP certificates you are ordering by adding the amounts you wrote in on the lines for each of the businesses for which you ordered SCRIP certificates. Write that total amount on the "Total $" line just to the right of your signature.
Then write out a check for the amount on your "Total $" line. Please make that check payable to "St. Patrick SCRIP."
In the bottom right corner of the SCRIP order form, there are 5 lines where you can choose to designate how you would like to get your SCRIP order once it has been filled. They look like this:
Send Home With Child___________ Name_________________________ Grade_________
Pick Up_______ Date:_________
Here you can designate a St. Patrick School student with whom we should send home your SCRIP order. That student may be your own child, or the child of a relative, neighbor, or friend who will be able to get that order to you. In that case, print an "X" in the blank next to "Send Home With Child," print the name of the child in the blank next to "Name," and print the grade level of the child in the blank next to "Grade."
There is also an option on the last line to tell us that you will stop by the school during the time the school is open to "Pick Up" your SCRIP order. In that case, print an "X" in the blank next to "Pick Up," and use the "Date:" line to indicate what date you will stop by to pick up your order. Your order will be held at the school for you to pick it up. Check at the main desk when you come to pick up your order.

In case you are wondering what the information on each line on the SCRIP order form means, here's the explanation. For example, here's a sample entry on the form:
Lowe's Home Improvement (3%): $25_____ $100_____
In this case, "Lowe's Home Improvement" is the business participating in our SCRIP program and offering certificates for sale.
The "(3%)" basically means that Lowe's Home Improvement donates 3% to our St. Patrick School from the total amount of Lowe's SCRIP certificates purchased.
The "$25_____ $100_____" means that Lowe's Home Improvement makes available for sale 2 different certificates -- one for $25 and one for $100. That way someone who is preparing to make some big purchases at Lowe's can purchase several of the $100 certificates to cover the cost of the purchase, while someone making smaller purchases may wish to purchase several of the $25 certificates instead.
